Sultan Fitness Liability Waiver

I, hereby acknowledge and agree to the following terms and conditions in consideration of being permitted to use the facilities and equipment provided by Sultan Fitness:
*         Assumption of Risk: I acknowledge that participating in physical exercise, using gym equipment, and engaging in fitness activities at Sultan Fitness involve inherent risks of injury, including but not limited to muscle strains, sprains, fractures, and other serious injuries.
*         Release of Liability: I hereby release, waive, discharge, and covenant not to sue Sultan Fitness, its owners, employees, instructors, and affiliates from any and all liability for any injuries, damages, claims, demands, or actions arising from the use of the gym facilities, equipment, or participation in any activities on the premises, including but not limited to negligence or failure to maintain a safe environment.
*         Assurance of Fitness: I affirm that I am in good physical condition and have no medical conditions that would prevent me from safely engaging in physical exercise. I agree to use the equipment properly and follow all safety instructions provided by the Sultan Fitness staff.
*         Indemnification: I agree to indemnify and hold harmless Sultan Fitness, its owners, employees, instructors, and affiliates from any and all claims brought by third parties arising from my use of the gym facilities and equipment.
*         Consent to Medical Treatment: In the event of any injury or medical emergency during my presence at Sultan Fitness, I authorize Sultan Fitness staff to seek medical treatment or emergency care as deemed necessary.
By signing below, I acknowledge that I have read this waiver, understand its contents, and voluntarily agree to its terms.
